It
has been almost 30 years since the documentation guidelines for office and
outpatient visits were written. As part
of the Patients Over Paperwork Act these outdated guidelines are being retired
and the American Medical Association (AMA) and the Center for Medicare and
Medicaid Services (CMS) have created new guidelines that go into effect for
dates of service beginning January 1, 2021
